💡About the Role:
As a Patient Care Coordinator, you will be a key communication point between patients, providers and clinical teams.

Your day-to-day work will involve appointment scheduling, patient communication, telehealth coordination, intake, referrals and EHR/EMR documentation.

Because the role supports U.S. healthcare operations, you will work Eastern Time hours, 8:00 AM–5:00 PM, Monday through Friday, including applicable company holidays. 🇺🇸


📌Key Responsibilities:

  • Schedule and reschedule patient appointments accurately.
  • Send telehealth links, appointment details, reminders and confirmations.
  • Manage no-shows and last-minute appointment changes.
  • Handle incoming patient calls and messages professionally and empathetically.
  • Route medication refill requests to the appropriate prescribing provider.
  • Direct patient requests to the correct department or provider and follow up on pending items.
  • Communicate provider instructions to patients as directed by clinical staff.
  • Assist with patient intake, registration and record updates.
  • Coordinate external referrals and follow-up appointments.
  • Document patient communications accurately in the EMR.
  • Escalate urgent or clinical concerns immediately to appropriate healthcare staff. 🚨

🎯Requirements:

  • High School Diploma or equivalent required.
  • Associate’s or Bachelor’s degree in Healthcare Administration, Communications or related field preferred.
  • 0–2 years of healthcare customer service, patient coordination or medical front-office experience.
  • Strong organizational and multitasking abilities.
  • Excellent attention to detail.
  • Professional, compassionate written and verbal English communication.
  • Basic Microsoft Office proficiency.
  • Comfortable using EHR systems, telehealth platforms and cloud-based tools.
  • Ability to work independently in a remote environment.
  • Ability to work U.S. Eastern Time hours.
  • Signed IRS Form W-8BEN required before engagement for non-U.S. remote workers.
